This place is NOT CLEAN! The floors, walls etc. are FILTHY. Saw cockroach in patient room. Anyone saying this place is clean is trying to bump up the rating. Attached is showing laundry tub that patients use to wash own laundry. This olace gets zero stars.

My mother in law was in this filthy place for three days. She was given insulin for diabetes and she’s not diabetic! The staff are nice but way under the level of care and experience to address the needs of the patients. Nurses AIDS pass out medication and they are not truly trained for this duty. Speaking of patients, one guy was out by the nurses station screaming at the other patients. The staff just laughed and let him bully them. The whole building is deplorable condition with worn out furniture and scratches and stains in the flooring. The county and state health departments should…

The county and state health departments should oversee this place before they harm anyone who deserves quality care. Shameful.

I went to see my friend today... and the smell was awful. This was by far the dirtiest facility.I have ever seen. If I could give it 0 stars, I would. How is the health department not shutting this place down?

No stars should be an option for this facility. Disgusted and appalled. Contacted protective services and allegheny county ombudsman. You can change the name all you want, but this place is ran by the same people and it is operated the same. Horribly.

This place is absolutely horrible!! Please think twice before sending your love one here!! My dad was place on 3-26-26 and by 4-13-26 he was already going back to the hospital for a bad bladder infection/ septic… He would go to the bathroom and they would like him sit in it close to 45min. He would call them and they would be so mean to him the he would call me all hours of the night asking me to help him. They didnt know if his wounds needed change 1x or 2x a day. I was told different versions of it. They dont use wipes only towels on them when needing change. They never did any of his stuff he needed coming into this facility. I was constantly calling and complaining on the level of care my dad was getting. Once i talk to the case manager she told me that the issues i had would get fixed so i gave them a second chance and what a mistake that was. My dad was sick all weekend prior to going to the hospital that i pushed for. They told me he was fine. Umm excuse me when does he has blood in his catheter and not any urine!! Apparently that wasnt a big deal. The head nurse there actually ask me what i would like her to do! I then said how about u call an ambulance. I will attach the pic of the blood in his catheter. Once my dad got to the emergency room they found out he has so much urine in his bladder that it caused a huge blood clot and his blood pressure dropped to 68/23… So yes i almost lost my dad due to their negligence!!! Needless to say he did not go back to this place!!! I made sure i had all his stuff when the ambulance came to get him that Monday!!!! If i could give them a zero i would!! They should be shut down for good!!!

Be warned. This place does not answer the phone and will not call you back.

This place is horrible. Employees just sitting around playing on their phones instead of taking care of the patients. My father got taken back to UPMC a few times, and every time he returned, all of his snacks we bought for him were stolen. They were even stealing his drinks while he was sleeping. Do not take your loved ones there

Do better with your staffing!!! One aide for the entire building is not only unacceptable but it is dangerous. It's the day before Thanksgiving so you should have known you would be short staffed. I had to come there and put my family member in bed because they had been sitting up for an hour because there was only one aide!!! DO BETTER ADMINISTRATION!!! I guess y'all don't care as long as y'all get to go home to y'all families. All the money y'all charge for a person to live there and y'all can't even provide adequate staffing so not only is the staff safe, the residents will be safe too!!!! Use agency staff if you have to just DO BETTER!!!
