I am writing to formally express my serious concerns regarding the care my mother received during her recent one-week stay at your facility. Unfortunately, her experience fell significantly below the standard of professionalism, dignity, and compassion that patients and their families rightfully expect. On one occasion, when my mother requested assistance to use the restroom, an aide entered the room and handled the situation in a highly inappropriate and disrespectful manner. The aide forcefully moved her wheelchair and abruptly placed a bedside commode in front of her, creating an environment that felt both hostile and demeaning. My mother, who is fully alert and capable of advocating for herself, addressed the aide directly and expressed that such behavior was unacceptable. In another instance, a staff member brought her medication but dropped it on the floor. Despite this, the staff member attempted to administer the same medication, which my mother appropriately refused due to sanitary concerns. Incidents such as these are deeply troubling and raise serious questions about both staff training and adherence to basic care standards. While my mother reported these occurrences, it is concerning to consider how patients who are unable to advocate for themselves may be treated under similar circumstances. There should be zero tolerance for any behavior that compromises patient dignity, safety, or well-being—especially when caring for elderly or vulnerable individuals. Additionally, it was evident that the facility is frequently understaffed. Notably, staffing levels appeared to increase only during a county visit, suggesting a temporary adjustment rather than a consistent standard of care. This raises concerns about the authenticity of operational practices and overall accountability. It is imperative that the facility prioritizes hiring and retaining staff who demonstrate professionalism, compassion, and a genuine commitment to patient care. Individuals who lack these qualities not only negatively impact patients but also contribute to a difficult work environment for those staff members who are dedicated and competent. I am not seeking a generic response, but rather meaningful corrective action. Acknowledging these issues and implementing sustainable improvements is essential. The consistency of similar concerns reflected in other reviews indicates that these are not isolated incidents, but part of a broader pattern that must be addressed. Patients should feel safe, respected, and supported during their stay—especially when they are in a vulnerable state and in need of care. It is deeply disappointing that instead of focusing on recovery, patients may feel anxious about how they will be treated when requesting basic assistance. I strongly urge your leadership team to take immediate and decisive action to ensure that all patients receive the respectful, professional care they deserve.

This place needs shut down! My sister was taken twice by ambulance from there due to their neglect and abuse! The staff is rude and ignorant and got an attitude of you asked questions! The place is a hellhole that I wouldn't put someone in even if I didn't like them! Then to try to get your loved one out is impossible! This has been an ordeal that has lasted almost 2 months ending in my sister's final discharge from mercy hospital with only weeks to live. Stay clear of this place!

Mistreated my kindhearted Aunt with the absolute most disgusting, abusive, and cruel behavior. Truly heartbreaking and unbelievable

First of all if your insurance can place you somewhere else, dont hesitate. This is an old building. All the patients DO NOT have seperate floors or sections. Patients were in hallways just roaming and sometimes asked me for help. My mom was in there for rehab and she was put on a floor and roomed with permanent residents, long term, short term or rehab care. Her roommates had the tv on 24 hours a day. Very loud. Needless to say my mom hardly got any sleep. The bathroom was not available at times and my mom had to be taken to other patients bathrooms to use the toilet. Her first bed didnt have a footboard and i cut my leg on the bolt sticking out of it. The other room she stayed in had a hole in the wall and her roomate had a overflowing personal items in boxes and bins. Nursing care was just adequate. Some nurses were more CARING than others. Came to visit my mom and came upon a nurse in the hall that was asleep at her station. Her co-worker slapped her leg when she saw me to alert her to wake up. Physical therapy department was good. House keeping was pretty decent. My mom's was in there for 10 days. I tried to get her out of the there but there wasn't any beds anywhere else to transfer.

I wasn’t going to share because this has been a personal struggle for my mom. Im sure no one ever wants to go to a nursing home. My mom gave up her apartment, her things, her comfy chair, her chicken pot pies. I go when I can because I love my mom so dearly. I know when I’m not there she is taken care of. Is it perfect? No- but show me one place that is. If I have an issue, I as her advocate, address and it is attended to. Wendy has been wonderful! I get called if there is an emergency. Please- unless a place is just truly awful don’t say unkind things. I have grown to love the staff at Reformed!

Unfortunately, my aunt has been to many similar facilities and this by far has been the worse. The staff were some of the rudest I have ever encountered. You know they really don't care when they act that way in front of the family. I spoke to Wendy the social worker and expressed concern about the staff and she down played it completely. That facility is trash, the entire building smells horrible. Do not take your love one's there please.

My experience has been great for my aunt in rehab , Gerald the nurse Shantell the nurse and Joe the occupational therapist are all very professional and seem to go above and beyond, I have a meeting with Wendy tomorrow and she seems very knowledgeable and professional. They all have an extremely hard job and all do it well. Thank you for taking care of my Aunt Eileen Conley.

Run away fast! Workers and city paramedics both told us, “you don’t want to be here.” It was a horrible setting. Understaffed. People are here to die and it’s frightening.
