I've struggled to write a concise review for several years. I hope there's been improvements since 2023, when my father was last a patient here. If 0 stars were an option, I would've chosen that. Some of my concerns are endemic to our country's rehab healthcare facilities. Many are understaffed and that staff is underpaid, and this was no exception. While he had at least one exemplary CNA caring for him, there was also one who was physically abusive towards him. More often than not, his needs went unmet. My father was eventually suspected of having a stroke and admitted to Doylestown and then flown to Jefferson. At Jefferson, it was discovered he was in sepsis due to a psuedomonas infection (a common bacteria in healthcare facilities and nursing homes, which we think he might have avoided if not for some observed, improper catheter procedures at Heritage). He'd also previously complained of shoulder pain, and Heritage told us they'd X-rayed and found no issues. In the hospital, he was found to have a broken clavicle. He was no doubt a difficult and grumpy patient, but staff should be prepared for that, given the age group they're dealing with. An investigation after his passing did not turn up anything of note, but the missteps we observed would be difficult to prove without video evidence/documentation (which I would recommend people recording if they have concerns for a loved one at this facility).

My mother has just been discharged today. This was the second time she was treated there. Staff was very helpful and informative. If necessary, I would definitely choose them again.

Very professional and very pleasant, no argument with the place. The physical therapy is great, when I came I couldn’t really walk but now I can. So I would say this is a great place.

I have serious concerns about this facility’s patient safety, communication, and overall care. Hallways and exits were frequently blocked with medical equipment, creating potential safety hazards and making it difficult for residents to move safely. During the stay, a medical issue worsened over many hours before emergency care was finally arranged. Communication from staff was also extremely poor, including an inability to provide clear information about a hospital transfer or transportation details. Based on this experience, I believe the facility needs significant improvement in emergency preparedness, care coordination, and patient oversight.

Heritage pointe is a great place. All staff are truly welcoming and nice. Highly recommend!

Great facility with knowledgeable staff who truly care. Would highly recommend!

I spent over 20 days in the facility and appreciate all the care and compassion I received from everyone in the building. They keep residents engaged with bingo, movies, games, and happy hours that include live singers. The PT/OT department is outstanding, the entire team is excellent, The therapy area is well laid out with quality equipment that supports both advanced therapy and simple exercises. The aides and support staff provide compassionate, professional care. Some challenges remain, as staffing issues appear to be systemic to the industry rather than specific to this location. At times, nurse-to-patient and aide-to-patient ratios make immediate attention difficult, despite staff doing their best. With stronger staffing support, this facility has the potential to be even better.

I would choose Heritage over and over again for my loved ones. The staff is very attentive and so are the aids. Rehab got my mother where she needed to be. I will forever be great full for all who help with her recovery from social worker to nurse and therapy!! Thank you! Everything was great!
