After 5 PM on Friday the service is horrible no one wants to answer the phone and the care for the patients declines because all the higher ups don’t work the weekends!!!
The care my loved one received was wonderful. I especially appreciate April RN, Billy RN and Cliff, respiratory therapy. Wonderful people.
Being harassed by a nursing home for a sibling's bills is a common issue, often based on illegal, high-pressure tactics. Under the federal Nursing Home Reform Act (NHRA), nursing homes that accept Medicare or Medicaid are prohibited from requiring a third party—such as a sibling—to guarantee payment for a resident's care.
This place has wonderful staff! I had to get physical therapy for balance issues. They got me back to being more independent.
I took advice to send my mom to this facility cause it was close to where i live and i can visit her, i made a big mistake taking her to this place. It's a dump, over crowded, over populated. Incompetence/unprofessional with no urgency and lack of caring descibes the workers best. My mom had to be sent to the hospital because lack of proper care and days later i still haven't heard from them or can get someone to return my call. If you care about you're loved one i recommend never sending them here.
They treated my dad very well. Very clean place
My husband Tom Rizor was at Transitions from December until June 21, when he lost his battle. This was the third Healthcare Facility in Washington that he had been in since July of 2024. I must say contrary to many comments, I found the place one of the best. The nurses, aides and every other worker was very friendly, ready to help with anything we needed and took such good care of Tommy. He had many different problems. Not an easy patient. You all made his last months as good as they could be. Hoping to stop back to see you girls.
It's a dump 😭 the care the med be careful that they give the patient the Med . Please don't take your loved ones to this place it's a nightmare 😭 😭 😭 😭
