I strongly recommend seeking an alternative facility for anyone considering Pathways Memory Care. Our family’s experience here has been deeply distressing due to systemic administrative failures and a severe lack of adequate patient oversight. While residing at this facility, my father suffered multiple falls because residents are frequently left alone for long stretches of time without any aides or nurses present. Furthermore, there was a critical failure in the maintenance and hygiene of his catheter. Because of this medical negligence, he contracted severe infections that resulted in two separate hospitalizations for sepsis. The frontline caregivers, nurses, and aides themselves are not to blame; they clearly cared for the residents and tried their best under difficult circumstances. The issue lies entirely with management. The facility has intentionally reduced its headcount, leaving the remaining staff severely overworked and stretched too thin to provide safe, basic care. Because management has failed to maintain safe staffing ratios, the standard of care has dropped to a dangerous level. My father’s health was severely compromised as a direct result of this lack of oversight. Please look elsewhere if you want a facility that ensures basic medical safety and adequate supervision.

they fired my grandmothers favorite caretaker of FOUR years over "neglect." She has never been more heartbroken. Considering moving her to a different facility.

My father recently moved into Pathways. I feel confident knowing he’s being well taken care of. The staff has made him feel welcome and they appear to truly care and enjoy their work. Thankful we came here.

My Mom had been at Pathways for over 2 years and the staff have always been so good to her. She unfortunately passed away 10 days ago, Nurse Esther kept in touch with me concerning the changes she was going through, it was so nice to know how much concern and love was given to her. Also, Mary Grace was there to provide any assistance needed concerning other personal issues. Eileen and Lisa are doing a great job, I will always be grateful for the support and guidance provided to me and my family. Thank you 🙏 Linda S.

As a healthcare provider who works closely with various assisted living communities, I have been consistently impressed with the quality of care and professionalism at the pathways. The staff demonstrates excellent clinical judgment, compassion, and respect for residents’ dignity. Communication with families and outside providers is timely and thorough, which is critical for continuity of care. The environment is clean, safe, and welcoming, with activities that promote both physical and emotional well-being.

This comminity feels like home with residents having fun and staying busy. There is a good resident to staff ratio and a variety of activities to participate in. I've heard the meals are tasty as well.

Nurse Prejudice … Everyone else was SLEEP and I was just sitting reading and because I’m agency the Nurse treated me like trash . Aggressively overbearing bearing , antagonizing me, put her Hands on me , called me “ agency girl “ I have videos too of her talking about me . And them sleep while she is right there. But harassing me . I did rounds like she said at 12:10 . She didn’t believe me . She put her hands on my shoulder and said you know you won’t be back right . I said don’t touch me . I left 2 times for a few minutes to cool off and get coffee . When I went to break said “I’ll be back in breaking” . Came back she handed me my phone at the door and said you can leave bye . I said I have to get my things . She told the other staff not to open the other door , Called the cops when I asked to be clocked out on the app ( which is policy ) I recorded the nurse saying not to let me get my things . Don’t put your parents here the staff does nothing but sleep . And take up for each other , And she called the owner and said they approved for the cops to be called instead of de escalating and clocking me out . She was smiling . God will get her at the end . This post shows the behavior and integrity of those . Wouldn’t trust especially the nurse . Lies

The Pathways at Warrington facility is modern and very clean. The administrative and nursing staff are caring, friendly and competent. They are prompt in responding to any concerns.
