There were significant concerns related to lack of meaningful activities, inconsistent communication, inadequate staffing, and poor coordination of care. Requests for engagement and basic support were frequently unmet, and responsiveness during medical situations were unreliable. The environment lacked accountability and oversight which appeared to negatively affect resident well-being. Families are encouraged to ask detailed questions, observe…

Requests for engagement and basic support were frequently unmet, and responsiveness during medical situations were unreliable. The environment lacked accountability and oversight which appeared to negatively affect resident well-being. Families are encouraged to ask detailed questions, observe closely, make surprise visits, confirm their ctivities chart, and meals listed. Also, advocate persistently when evaluating care options at this facility.
This place has a very nice staff. They are polite, pleasant and hard working. Even when they are short staffed, they keep a smile on their face. This combination makes for a winning team. They are often short staffed resulting in a greater workload which can be overwhelming to the staff. The communication between staff is poor. They don't seem to have change of shift report. Additionally, they often say yes to family requests, but rarely follow through often forgetting. Perhaps because they are often short staffed. They are often short staffed resulting in a greater workload which can be overwhelming to the staff. The communication between staff is poor. They don't seem to have change of shift report. Additionally, they often say yes to family requests, but rarely follow through often forgetting. Perhaps because they are often short staffed. The residents do not have a call button or means for reaching staff should an emergency occur, like a fall. To address this, the staff are suppose to frequently check in on the residents. These rounds are are inconsistent, and do not occur on a regular basis. Making a phone call from the outside, and having someone pick up is nearly impossible. Voice messages are rarely returned. Regarding the building? It is sad and decrepit. The lights are low giving it a dull gloomy environment. The floors are dark, worn, and outdated adding to making the environment somber. The walls are stale and old looking. The bedroom are the same. Recently the walls were painted. What a difference paint makes! However, only the entrance was done, the hallways remained the same. This gives the impression that "it was all done for show." The food? Simple, bland, and average: hot dogs, burgers, sloppy joe's, Mac & Cheese. The outside entrance and back are nice. To the owner of Bethlehem Manor. Please update the building. ,paint ALL walls with bright colors and updated wall pictures.
Make sure the staff is fully aware of food allergies. My father was deathly allergic to shellfish and on a day I was visiting (thankfully I was there) they served him shrimp. The staff is sweet but they are short staffed and they become overwhelmed, especially in memory care. My father had soiled himself during a visit and it wasn't until I offered to bathe him myself nearly 20 min later that someone was actually able to help. He had several falls and during a visit was left unattended in the locked dining room. I recommend not letting staff know when you're visiting and visit at random…

The staff was wonderful . They went above and beyond to make my wife feel loved and comfortable . All were great people . They treated her with respect and care. Health and cleanliness of the residents was always foremost in their daily duties
I would recommend to others. All the staff has been super nice to my mother and it's a good fit for her. Their memory care unit was the most reasonable in the valley and we needed that.
I placed my father, [name removed] at Bethlehem Manor when he was no longer able to care for himself. [name removed] was always a difficult person. Demanding, controlling, paranoid, accusatory, etc. The staff there treated him with dignity and respect. They worked around his idiosyncrasies and kept him clean and comfortable.
My father has moved into Bethlehem Manor. It's a pretty great place. The staff is very nice. Everybody is coordinated well, so we have no issues. If I have any issues, they usually resolve it immediately. My father is not too good with the cellphone so they Facetime me there pretty much with their Bethlehem Manor's phone, so I'm able to see my father if I'm not able to make it, which is great. It's a really good place. My father has a little studio. He has his bed set up. He has a TV wall-mounted. He has a mini-fridge. He has a wheelchair, so he can pretty much roam around there. He has windows, so he can look outside as well. So it's pretty open, and it's not like he's cramped or anything, which is great. For amenities, they have a large dining room area and an outdoor area, which is more of a patio. They have game nights and music. They have a lot of different types of activities. So my father is kept very busy.
I moved my husband bc the care was so insufficient. He lost 20 lbs in one month, was over medicated at time or not medicated for pain, had soaked diapers almost daily at evening time where he wasn't rounded on all day, and was left in his chair mostly every day ALL DAY. He could walk when he went there and when I pulled him he could hardly stand up. He's been in the new facility for only 5 days now and is getting balanced home cooked meals daily 3x a day as opposed to getting just mashed potatoes with a little minced type of meat in it for lunch AND dinner daily. There were NO ACTIVITIES and very UNDER STAFFED. Only one aide on mostly every evening and one med tech that took care of both floors. She always did the 1st floor first bc the pts were more alert and functioning so that was easier and by the time she got upstairs to the dementia ward meds were always late. The place was not kept clean and they lost several of my husbands items of clothing. I could go on but I won't! I compare the care there to that of a poor mother dog locked up in a cage spitting out puppies with no regard to care, health or age and filth. If you want honesty you just got it. I'm an RN and I know the difference between good quality care and deplorable. Some of the aides were very nice and a few of the med techs as well, but they have no say so in scheduling and what is allowed or not. The place should be closed down.
