Please don't let your love ones go to this place. The place is dirty and the room they call a beauty shop is like a filthy closet. Please don't let your love ones go to this place. The place is dirty and the room they call a beauty shop is like a filthy closet. It had hair all over the floor and trash that looked like it had been there for months. My friend was at this facility for 3 wèeks and never had her night gown changed. When she left this facility they only sent 3 of her shirts and one pair of pants that were in her closet. I had taken over at least ten pairs of pants and about thirty good shirts. Two jackets were also missing. Her personal toiletries missing. All new clothes and toiletries had to be purchased. I called several times to complain and asked them what happened to her belongings and they had no answer and just kept saying they would look into it and call me back. They never called back and the lady who supposedly was in charge would never answer her phone when I called. Patients would not be fed if they were sleeping at meal time and would not be woken to be fed. My friend is in another facility and very happy in her new clean and beautiful home. If I could give it a zero star I would.

Just over all bad. Workers were slow and place was not very clean. My fathers room was dusty and dirty. The bathroom was not cleaned. There was nasty stuff in his bathroom. That was there from the last time I visited. These workers are supposed to be here to take care of patients, not treat them bad. And, the people.who clean need to do a better job and start checking bathrooms and garbage. Will NOT recommend this place for anyone who needs to fund a Healthcare/rehabilitation center.

I am absolutely blown away by the incredible care and attention my mother has been getting at Beaver Healthcare and Rehab! My whole family was hesitant to send our Mother to a facility but we knew fairly quickly that she was in good hands! The staff went above and beyond to make her feel comfortable and at ease. From the nurses to the therapists to the activities women, everyone was friendly, knowledgeable, and compassionate. The facility itself is much cleaner and well-maintained vs other facilities I have seen in the past. The rooms are cozy and comfortable, with plenty of natural light and…

The rooms are cozy and comfortable, with plenty of natural light and fresh air. They have a nice big dining room where we were able to have the whole family visit. But what truly sets this facility apart is the level of care and dedication shown by the staff. They genuinely care about each resident as an individual, taking the time to get to know each one and using that info to provide the best care they can. They celebrate the small victories, like when my mother was able to get herself in and out of bed. I was worried about leaving our mother but honestly it has become a second home to her. We are all just so grateful for the care she received and would highly recommend this facility to anyone in need of skilled nursing care.

The care to say the least, was substandard, to say the most, negligent and abusive. I was there for physical rehab, because I was malnourished and had lost partial use of my legs. On one occasion, I had rung the call bell, and when no one came, I went to the restroom alone. I was supposed to have assistance. I fell leaving the bathroom. I called out for help, and a nurse named Maria came in the room. She immediately began to berate me for having fallen. And said "get up, use your walker *you're not paralyzed, stand up". "I'm not a crane, to lift you....I have patients all up and down the hall....my back hurts , and I'm not going to lift you." I crawled to the bed and tried to lift myself up. I couldn't. She came over to the bed still yelling, and lifted me up by my pants and onto the bed. I needed nausea medication, because the abuse made me feel like I was going to vomit. Don't subject your loved ones to this place. Most of the staff are rude and uncaring. I filed a report with the administrator. Nothing was done as far as I know regarding the incident. The exception to the neglect/abuse, was the PT/OT department. They were outstanding, and caring. I can walk with a walker because of them. I will always be grateful to them for all the support and assistance they provided.

I've been here at Beaver Healthcare and Rehabilitation for 5 months now. It's a pretty large facility, and it's all on one floor. Right now, there are about 35 residents here. They have a large dining room, but you can eat in your room or the dining room. They have a few different activities like coffee and chat in the morning, jeopardy, puzzles, bingo, and outdoor time. My room is supposed to be shared, but I've been alone in it for most of my time here. But they have private rooms, too. Some staff are very nice and very helpful, and then there are some that are just here to work. The physical and occupational therapies are OK, but not the best.

My mother went there for physical therapy, luckily she was only there for 3 weeks but still that was 3 weeks to long. The day she arrived there from the hospital I took her clothes and hygiene products. When I walked in the room I was shocked. The TV was old enough to get served in any bar, the room was small and she had a roommate. Also, her roommate and her had to share this TV, I'm not kidding it was the old big thick style, and it was like a 27 inch. The staff would show up eventually when the bell was hit but they weren't in any hurry. The food was subpar. A few of the brand new clothes went "missing" and they had her name written in them.

Completely disorganized and unprofessional. Most workers are so disrespectful, rude, miserable, unfriendly, and unaccomodating...only there for a paycheck. A few were caring but the rest need to find other jobs because they don't care about a person's dignity or well being. Not helping a patient to the bathroom, that is capable with assistance, and letting them sit in a diaper or on a bed pan. Throwing urine soaked clothes on the shelf in the closet or an empty drawer. The whole room smelled. Then...leaving a soiled brief inside pants instead of throwing it away. Just lazy, incompetent, ignorant, disrespectful and disgraceful. Don't ever let your loved ones go there...you will regret it.

This facility has the best CNAs I’ve ever seen. They all work so well together and they are great with all of the residents.
